I would give it some more time to tell 100%. Claws or tail not fully extended out. Photos not very clear to see base of claws which could offer more info. How long this has been going on ? What ever the time been that's time lost for proper treatment ! This is not common. Most cases of this turns out very bad. See a Herp Vet now or at least a vet that knows turtles ! Swelling is normally from infection and once in blood stream , fatal in most all case...

Nothing you can't fix. Those closed type basking areas with the environment turtles needs makes a nice home for all kind of things. Some don't clean it when cleaning there filters. I rehabbed turtles at the center with scute's missing and done very well. Turtle are very hardy.

What test was done to identify "RI" by the vet and was he/she a Herp Vet? If he/she actually treated turtle's before should have given clear cut instructions for the turtle , exactly what was them if any? What did he/she prescribe ? To me the skin patterns are off some , at least what I see of them. Although patterns do change with growth , especially in aged turtles. Also the scute's have a color in the pattern in areas. If it was cleaned up could tell for sure. Way too much algae. Could be ? Sex , would have to see the cloaca...
